What is Social Prescribing? What is Self-Care?
KCSC currently manage two Social Prescribing self-care schemes and a Directory:
- My Care My Way
- Healthier Futures North Kensington
Social prescribing works alongside and supports traditional medical interventions by linking people up to voluntary and community groups and other non-clinical services. Social prescribing is a way to help people make positive changes in their lives by giving them the knowledge, skills and the confidence to do so. Self-Care is intentionally taking time to care for your physical, mental and emotional health.
Until 2024, KCSC also ran the self-care element of the Community Living Well programme. A report on the self-care programme will be available soon.
My Care My Way
My Care My Way (MCMW) is a programme for anyone aged 65 and over. It is for people registered with a GP in Kensington & Chelsea, Queens Park or Paddington.
Healthier Futures (North Kensington)
Healthier Futures is dedicated to supporting the wellbeing of North Kensington residents affected by the Grenfell Tower fire.

The Social Prescribing (Self-Care) directory lists local community services that residents can access to support their health and well-being.

Case Studies
In 2023-2024, over 3,000 people were referred to self-care services. Since the program began, there have been more than 10,000 referrals and over 30,000 sessions delivered. Below are recent case studies showing how these services have positively impacted local people.
